ATHLETICS - IMPORTANT INFORMATION: All students intending to participate in athletics during the 2008-09 school year must read and understand the following information. Failure to complete all the required paperwork prior to the first practice will prohibit you from participating until your paperwork has been completed, submitted to Mr. Johnson, the athletic director, and an Athletic Card has been issued. Do not attempt to leave your completed paperwork with anyone except Mr. Johnson or Tiffeny Beyer in the Athletic/Activities Office at AMHS.

To be eligible to participate in athletics during the 2008-09 school year you must have a new medical examination completed after July 1, 2008. You must complete an Auburn School District Athletic Registration Form with parent signatures in three locations. Once to grant permission to participate with the risk of injury, once with insurance information, and once to give permission to provide medical care and treatment. There is also a section provided for your doctor’s signature after the physical has been completed. A new ASD Athletic Registration Form is being developed and should be available before the end of this school year. Students should be able to pick one up from their school, their coach, or on the ASD website. The final item necessary for athletic participation is proof of purchase of an AMHS ASB Card. The cost of the card this year is $45.00 and can be purchased at the AMHS Bookkeeper’s Office. The bookkeeper’s office hours are 7:00 a.m. to 3:30 p.m. daily. We have established the following dates and times for athletes to submit all paperwork to secure AMHS ATHLETIC CARDS:
